Community Health Needs Assessment

PHRG is a pioneer in designing and implementing health needs assessments throughout the nation. With a grant from the U.S. Centers for Medicare and Medicaid Services (formerly HCFA), we developed the Community and Institutional Assessment Process (CIAP) in 1989. It is a proven planning process that identifies critical health services needs through quantitive health indicators.

The Community and Institutional Assessment Process (CIAP) uses scientifically validated indicators and outcomes of health status, health care and social service needs in a state of the art planning process. The CIAP guides communities and providers in benchmarking rates of health indicators and outcomes, understanding current health care use patterns, accurately planning for future demand of health care services, and making informed and effective delivery system changes. The result is strategic health services planning that improves the financial viability of providers and the health of the people they serve.

How the CIAP Works

PHRG developed the Community and Institutional Assessment Process (CIAP) as a unique, data-driven population based planning system that combines epidemiological modeling, health services need/demand analysis and local health services expertise. The CIAP uses epidemiological and medical care utilization data from multiple sources, health outcomes data, peer group standards for comparison, and input from health care providers and community leaders. We apply our experience and customized software to identify, interpret, and project health care utilization patterns and service needs. PHRG converts this into information for health services planning, monitoring, and quality improvement.

Benefits to a Community or Provider that Employs PHRG's CIAP:

Improved Understanding of Community Health Care Needs
Using the CIAP, key health problems and health care needs of distinct populations and subgroups are identified. The CIAP provides objective information for assessing the strengths and weaknesses in the local health care delivery system. This results in recommendations by service line for prevention, detection, treatment and follow-up care.

Improved Strategic Planning
The CIAP is able to focus on those services that respond directly to community health status problems by understanding the health service needs of a population. The CIAP furthers this planning by providing clear, attainable goals, and measures of the actions implemented to improve the health status of communities and the financial viability of local providers.

Improved Key Resource Allocation Decisions
The CIAP supplies the critical information required to focus limited resources for maximum results. The information provided through the CIAP can support plans for targeted service expansion and reorganization of the health care service lines. The CIAP also identifies where expansion would be inappropriate.
