The Community-Based Health Services Linkage Tool is

- Assess the extent to which community health care needs are met by existing resources
- Identify additional resources needed
- Identify how existing resources can be better linked to primary care practices and patients for prevention, diagnosis
and treatment
The Patient Barriers to Treatment and Self Management assessment will:
- Identify patient care access issues
- Identify self management knowledge, skills and attitudes
